Referral Process

We would like to take this opportunity to wish you a very warm welcome to our dental clinic.

Your dentist has referred you for endodontic treatment and you will find enclosed with this letter an information leaflet about endodontic treatment. You will be treated in our centre by Dr Gerald O’Connor.

Treatment is usually carried out over two visits which are normally about two weeks apart. On some occasions it is not possible to do this, but this in no way affects the long-term prognosis of the treatment. You will return to your general dentist for the final restoration of the tooth, which we would recommend, is placed within two to four weeks of the completion of the endodontic treatment.

Your dentist will have already advised you of the fees for the treatment but if you have any enquiries please do not hesitate to ring the practice on (071) 916 0953 and ask for one of our assistants Jane or Catherine.  Please note that all endodontic treatment is carried out on a private basis and fees are payable on your first visit. Please note we accept all major credit and debit cards.

Enclosed with this letter, you will find confirmation of your appointment time. If you are unable to attend the appointment, it would be greatly appreciated if you could contact the practice at your earliest convenience.

• A medical history form (PLEASE COMPLETE PRIOR TO VISIT)
• An information leaflet about endodontic treatment
• A list of our fees
• A copy of our terms & conditions (PLEASE READ THESE CAREFULLY)
• A consent form (PLEASE READ BEFOREHAND AND BRING ALONG TO YOUR FIRST VISIT)
